Fear of Needles?

Watching an old episode of The Sopranos tonight from Season 3 when Christopher became a made man, I wondered something.

To become a made man, there is a ceremony when the guy gets pricked in the finger with a needle to draw blood. What if a guy did years of paying his dues, killing others to get the Boss his money, but he had a fear of needles and didn't want to be pricked?
